Patients can use OneChart to request, store, and share their medical records regardless of where those records are held. Your health information is securely stored in document form and is fully searchable. Simply list your providers and submit your records requests from within the app to get started.

The records requests subscription is auto-renewable which means that once purchased it will be auto-renewed every month until you cancel it 24 hours prior to the end of the current period. Duration of the subscription is 1 Month with a charge of $1.99 every month. Your first month is free. Your iTunes Account will be charged for renewal within 24-hours prior to the end of the current period with the cost of $1.99. Manage your Subscription and Auto-Renewal by going to your Account Settings.
